Several secretion products of the epididymis are found in seminal plasma, but the concentration of the enzyme neutral alpha-glucosidase was shown to be the most sensitive marker of epididymal function. A very low activity of alpha-glucosidase (<13.5 U/L) is found in cases with azoospermia due to obstruction of the deferential ducts.
Lower than normal values (<15 U/L) occur in patients with chronic inflammation of the epididymides (epididymitis) or in hypo-androgenism.
Assessment of alpha-glucosidase in seminal plasma is recommended in order to evaluate epididymal function in cases with asthenozoospermia, or cases suspected of chronic epididymitis. In men with azoospermia and normal testosterone concentration in serum, a low alpha-glucosidase activity suggests obstruction.
